If you experience or notice an employee of Summit Medical Group who has "GONE THE EXTRA MILE", please take a few minutes to show your appreciation and recognition by filling out an EXTRA MILE form. You'll find blank EXTRA MILE forms throughout each of our Summit Medical Group offices or you can send one online. The "EXTRA MILE" form is available to encourage colleagues, patients, families, associates, physicians, vendors or other customers of Summit Medical Group to acknowledge outstanding acts of kindness or extra efforts by any Summit Medical Group employee.

To ensure the employee receives the recognition he or she deserves, please try to submit the first and last name of the employee and the department or area in which they work.
